Delete a request but not removing it from the list
Delete a folder but not removing it from the list
Folder created, but doesn't appear in the list
Delete a workspace, but not updating it right away
Crtl + Shift + R = need force refresh
I haven’t been able to reproduce this. What OS are you using? Also, do you see anything in Devtools? (Right-click > inspect element > console)

I've done some testing and it seems that if the URL is empty, it's not deleted.
